Breast Lift Surgery OR Mastopexy

Breast Lift Surgery or Mastopexy is the plastic surgery mammoplasty procedure for raising sagging breasts upon the chest of the woman; by correcting and modifying the size, contour, and elevation of the breasts. In a breast-lift surgery to re-establish an aesthetically proportionate bust for the woman, the critical corrective consideration is the tissue viability of the nipple-areola complex (NAC), to ensure the functional sensitivity of the breasts for lactation and breast-feeding.

What does breast lift surgery do?

The surgery tightens the breasts and raises the position of breasts on the chest wall, in proportion to the upper body. It repositions the nipples and also reduces the areola region if it is large in proportion to the breast size.

The surgery is helpful in correcting pendulous breasts with large volume and those with breast asymmetry.

The surgery does not increase the breast size. For increasing the breast size the surgeon may combine breast augmentation along with breast lift surgery. Mastopexy gives a complete makeover to women’s breast by increasing its firmness and contour, and imparts a more youthful appearance.

Breast lift surgery can be combined with breast augmentation or breast reduction or breast reconstruction surgeries as is desired by the patient.


How is the Breast lift surgery done?

A complete medical and psychological assessment of the patient is done prior to the surgery and realistic outcome of the procedure is explained to the patient.

The surgery is done under general anaesthesia in which the patient has no knowledge of the procedure and is completely unconscious.

The incisions are usually made under the areola along the breast crease so they are less visible and blend with the natural creases. The excess skin is removed from under the breast. The breasts are remodelled and shifted higher. The nipples are also repositioned higher to match the position of the tightened breasts.

In case the female wants an increase in the breast size, then implants can be inserted along with breast lift surgery.

Non- surgical methods for breast lift like laser lift and toning machines are available in the market but are not very reliable as the results last for a very short time.

Recovery after surgery

Since the surgery is carried out under general anaesthesia, the patient will have to stay in the hospital for one night. The breasts will be painful and bruised and covered with gauge dressing. A well supportive and fitting bra helps to reduce the discomfort. Pain killers to be taken only on doctor’s advice.

Normal routine activities can be resumed 2-3 days after surgery, but strenuous exercises and activities should be deferred till 2 weeks at least.

The effect of surgery lasts for several years, however aging and gravity can causes sagging breasts again. Women generally opt for this surgery after their family is complete.


Placement of Implants:

Implants can be placed in 4 different ways:

Subglandular: the implant is placed just underneath the mammary gland. This results in higher chances of capsular contracture and is not suitable for thin women.

Subfascial: The implant is placed between the fascia covering the muscle and the pectoris muscle (chest muscle). This position gives better fixation of the implant and less movement.

Subpectoral: The implant is placed between the layers of the pectoralis muscle. Since the incision requires the muscles to be cut and mobilised, the healing time is more.

Submuscular: the implant is placed under the muscle layer and is generally preferred in breast reconstruction. Recovery time is more.

Types of Incisions:

Inframammillary Incision: this is the most common incision and is made under the breast. The scar is not visible as it comes in the natural crease. It allows maximum room to insert the silicone gel implant.

Periareolar: The incision is made around the areola (dark area surrounding the nipple) on the inner side. It is a small incision and used only to adjust the implant position. It is done when breast lift is also required. The incision is not very popular as there may be damage to nerves and breast feeding is affected. Also the chances of capsular contracture are more.

Transaxillary: The incision is made vertically in the armpit crease and the implant is tunnelled through horizontally

Transumbilical: The incision is made around the navel and the implant is pushed through vertically upwards. Not very popular as the implants may break while pushing.

Transabdominal: the incision is made in the abdominal wall and the implant is pushed through upwards. Again not a very popular surgery.

The surgery is carried out under general anaesthesia. The choice of implants and the type of incision and placement depends on the requirement, patient’s anatomy, medical condition, surgeon’s expertise and surgeon- patient relations. The surgery takes around 2-4 hours.


Types of Incisions:

  •   The scar requires around 6 weeks to heal.
  •   The patient is advised against any strenuous exercise.
  •   Complete range of movement at the shoulder and breathing exercises are a must.
  •   Sub-muscular implants require more healing time as the incision is long and the muscles are cut.
  •   Pain killers as advised can be taken in cases of severe pain.
  •   Normal routine activities can be resumed after 10 days of surgery.
